Refractory Angina Pectoris: Focus on Cell Therapy
نویسندگان
چکیده
Although medical and surgical treatments often provide adequate solutions for individuals with coronary artery disease, an increasing need exists to develop treatment modalities for those patients with angina who are unresponsive to medical therapy, have serious coronary atherosclerosis, and are not eligible for percutaneous techniques or bypass surgery, a condition known as refractory angina pectoris. In recent years, alternative experimental therapeutic options (e.g. ranolazine, enhanced external counter-pulsation, shock waves) have been proposed in order to alleviate symptoms and improve the quality of life in these patients. A promising therapeutic option arising from basic research is the use of autologous cells directly inoculated into the ischemic heart. Preliminary clinical trial results are encouraging. Aim of this chapter is to review the current knowledge on refractory angina, focusing on cell therapy from a biological and clinical standpoint.

Nonconventional therapeutic modalities in refractory angina pectoris
Abstract Despite sophisticated conventional medical treatment and the continued development and improvement of coronary revascularization modalities in recent years, a significant number of patients still suffer from refractory angina pectoris and cannot be successfully managed. Advances in therapeutic options have led to an increase in the average of life expectancy even in such ...
متن کاملComparison between Revascularization and Optimal Medical Therapy in Patients with Stable Angina Pectoris
Background and Purpose: Regarding usefulness of revascularization versus optimal medical therapy in patients with stable angina pectoris, data are challenging. The aim of this 12-month follow-up study was to compare the survival benefit associated with revascularization versus optimal medical therapy on the patients with stable angina pectoris. Materials and Methods: A prospective clinical s...
متن کاملNarrowing of the Coronary Sinus: A Device-Based Therapy for Persistent Angina Pectoris.
Alongside the remarkable advances in medical and invasive therapies for the treatment of ischemic heart disease, an increasing number of patients with advanced coronary artery disease unsuitable for revascularization continue to suffer from angina pectoris despite optimal medical therapy. Patients with chronic angina have poor quality of life and increased levels of anxiety and depression. A co...
متن کاملTreatment options for refractory angina pectoris: enhanced external counterpulsation therapy.
Refractory angina pectoris, defined as angina refractory to maximal medical therapy and standard coronary revascularization procedures, remains a significant health problem in the United States and the world. Despite a panoply of recent therapeutic advances, patients with refractory angina pectoris are not adequately treated; therefore, scientists have been investigating new technologies to hel...
متن کاملExtracorporeal cardiac shock wave therapy: first experience in the everyday practice for treatment of chronic refractory angina pectoris.
Several alternative therapies have emerged for treating patients with chronic refractory angina pectoris, yet only a few have given rise to sufficiently published data regarding safety and effectiveness. It is imperative to establish an effective and preferably non-invasive therapy for this expanding patient cohort. We report initial experience with cardiac shock wave therapy (CSWT) in the ever...
